Please note that the following form has now been withdrawn from use.

Any referrals submitted after 20 March 2026 will not be able to be processed.

After this date no new referrals/submissions can be made until the new replacement school portal goes live on Thursday 16 April 2026.  

If your school has not yet completed the School Portal User Form  to request access for a designated administrator, please do this as soon as possible to avoid any delays in future referrals.
